While Sean is sitting in his kitchen cutting the “40” off of his left over birthday cake in half. One cupcake with a “4” candle and the other with a “0” candle. Christian is alone in his apartment with his newborn son blowing out the candles on two cupcakes.
The closing scene is split between Sean and Christian. Because of his anxiety or “yip” as they call it, he truly believes that he can now barely get through surgeries. The episode ends with Sean and Christian arguing about their success in the future. During the consult, Christian decided to sleep with Sean’s mother in law to assure her that she didn’t need the surgery to feel young and beautiful. Sean refused to work on his mother in law because he saw too much of his wife in her, so decided to let Christian perform the surgery. The second client they take on in the episode was Erica Naughton, Sean’s mother in law.
He was moving forward with his life, while she was not, and at the end of the episode when he come in to visit her, she shoots him and I guess that was her revenge back at him after all these years. Which is why her face was scarred and his was not. They were both supposed to shoot each other because they realized they had nothing to live for, but when it came time to shoot, he shot her and she “loved” him too much to do it.
Throughout the process of the woman’s surgery, we find out that back in the day her and her best friend held the gun up to each other. McNamara explains to the woman that she will have to have five, if not more reconstructive surgeries in order to have her face back. The entire right side of her face is wounded and scared up from the gunshot. Her and her gay best friend come to the consult together and they talk about being successful at a certain age. The first client they take on is a woman who accidentally shot herself at a party. He is willing to change his sneaky and promiscuous ways to become a better father to his newborn son Wilbur. On the other hand, Christian starts to question himself becoming 40 as well. The doctor lets him know that he is in fact anxious and overworked. Christian suggests Botox but when he absolutely refuses, he goes to see a doctor instead. He feels that everything might go downhill from this point on. Sean is starting to feel anxiety about the way he looks and feels about being 40. Sean McNamara and Christian Troy both turn 40 and the age is starting to concern the both of them. This episode, like every episode, reflects upon their personal lives as well as the surgeries that they have to perform. I really didn’t like this episode, especially for a season premiere, in my opinion it was pretty weak. Sean McNamara and Christian Troy went through college and medical school together and now their dream of being plastic surgeons have become a reality. Two doctors, partners, and best friends own a private plastic surgery clinic.
